When you use an iPhone for the first time, whether it's a new one out of the box or a used one from another user, you must activate it in order to set up cellular service. However, if you want to take advantage of all the features offered by your carrier, you'll need to activate the phone.
Most carriers offer some type of subsidy program where they will pay part of the price of your phone up front and then bill your account for the remaining cost over a period of time. This allows them to provide you with a device at a discounted rate and also gives them room to negotiate with phone manufacturers for better deals.
If you're using an iPhone that came pre-activated from the carrier, you won't be able to switch providers until after your contract expires. Even if you decide not to renew your contract, you can't switch before then because you'd need to contact customer service to have your account activated by your previous provider.
However, if you buy an unlocked iPhone from another user, you can choose any carrier who sells prepaid plans. These carriers include companies such as AT&T, T-Mobile, Verizon, and Sprint.

If you have an existing iCloud account, just log in with your email address and password. If you don't have an existing account, you can create one by providing an email address and selecting a password. After activating the new iPhone, you'll be taken to the Settings app where you can manage its features. From there, you can also control which services and apps are allowed access to your iPhone via iOS Settings.
